Patients more than 15 min late will need to reschedule.
As a courtesy to others: we reserve the right to reschedule your appointment if you are more than 10 minutes late for your scheduled appointment time. Please allow additional driving time so you arrive promptly for your scheduled visit.
In addition: if for some unforeseen reason you find it impossible to keep a scheduled appointment; please contact us 24 hours in advance. We reserve the right to charge at least $50.00 for broken or cancelled appointments no matter the reason. Additional fees may apply for extended or group appointments.
We accept ALL major PPO dental insurances; including Delta, Metlife, Blue Cross Blue Shield,Blue Care Network, Aetna, Cigna, Humana, Guardian, United Healthcare, ADN and Dentemax Network.
We are unable to accept and do not participate with MEDICAID, HMO or DMO plans.
No insurance? or Senior Citizen? We can provide discounts for you.
